The movie picks up where the original left off, following a young woman named Heather Miller who inherits a mansion from a grandmother she never knew she had, only to discover that the chainsaw-wielding Leatherface still resides there, Picking up after the events of the 1974 original film, Leatherface is spinning around his chainsaw, still pissed that Sally got away in the truck, he turns off his chainsaw and heads back home, at Leatherface's home, Mayor Burt Hartman is arguing with Drayton telling Drayton he can give Leatherface a fair trial for the crimes he and the family committed, Drayton refuses. back at the police station, Sally arrives there and asks to speak to Sheriff Hooper, she tells Hooper what happens, Hooper gets pissed and has townspeople go over to the chainsaw house, back at the chainsaw house, there is a lot of townspeople and mob including Sheriff Hooper and Burt Hartman, the townspeople throw molotov cocktails in the house, burning the house to the ground and killing almost everybody in the house. Twenty years later, there are five friends, Heather, her boyfriend Ryan, Nikki, Kenny and Darryl are all in Newt, Texas going to a mansion. They arrive there and they all walk inside there, They check out the mansion, Heather, Ryan, Nikki and Kenny all go to a local grocery store while Darryl stays at the mansion, he goes upstairs and opens a door, he walks downstairs and opens a secret door where he on accident frees Leatherface, he sees Leatherface making a mask, Leatherface turns around, grabs his chainsaw, Darryl freaks out and tries to escape but it is too late as Leatherface runs up to him and slices his scalp clean off, killing him. about 30 minutes later they return back to the mansion, Kenny goes upstairs and goes down the stairs of the basement where Leatherface pops out of nowhere and knocks his teeth in with a hammer and brutally chainsaws him through the chest, Heather is still trying to find her grandmother's grave, freaked out though, Leatherface comes upstairs and comes face to face with her, he then tries to kill her but she runs as fast as she can, she screams and warns Nikki and Ryan, Nikki and Ryan run with Heather, Heather is lead in to a slaughterroom where Kenny's body is sliced in half and Darryl's head is chopped off, Nikki and Heather scream to the top of their lungs and Ryan yells "FUCK! Ryan, Heather and Nikki run all the way to the car, Ryan prepares to fight off Leatherface but Leatherface drops his chainsaw and stabs Ryan in his stomach, presumably killing him. Heather starts to cry and scream, Heather and Nikki hide in the back of their van, Nikki starts to cry and screams a little but Heather tells her she needs to be quiet, Leatherface picks his chainsaw back up and breaks through the glass with his chainsaw, Nikki and Heather scream, Heather and Nikki run out of the van, Nikki runs right in Leatherface's way, she is then gutted by Leatherface with the chainsaw, Heather is running as fast as she can, Leatherface chases after her to a carnival where he tears down the fence, he kills some of the people at the carnival, that's when Sheriff Carlton Hartman shows up and saves Heather by shooting Leatherface in his face,wounding him. Leatherface turns off his chainsaw and runs away. Sheriff Hartman lets Heather in to his car where he asks Heather numerous questions about Leatherface, Heather says she doesn't know what the hell is happening and why the son of a bitch is chasing her, Sheriff Hartman takes her to the police station, they both exit the car where she meets Sheriff Hooper, Sheriff Hooper and Carlton explain the story of what happened with Leatherface, Heather says it's a bunch of bullshit, she's going to avenge the town's brutality with Leatherface, Hooper and Hartman tell her are you really going to go with that maniac after what he did to your friends, he's your cousin. Heather says to Hartman "I will not be joining Leatherface, i will be the one killing my cousin. Heather goes outside and takes a police car where she arrives at the house what was burned down twenty years ago, Farnsworth and Leatherface are waiting for her, Farnsworth tells her I knew one day you would join this family. Heather then joins them, tricking the family. The police show up at the house where Ryan is still alive, Heather tells Leatherface to stop all the chainsaw killing, it is not you who is the people killing people, your being set up by these people cousin, just go to jail, this family is a bunch of bullshit, Leatherface then stabs Heather in her stomach with his meat clever, Heather then looks at Leatherface and tells him "you would kill your own cousin, Leatherface then looks at Heather and says to her "i didn't know you were my cousin. he pulls the meat clever out of Heather and starts to cry, Farnsworth then tells Leatherface to grab his chainsaw and kill all the officers, Leatherface refuses as he would rather die with his cousin than to kill with his family, Farnsworth calls him a dumbass retard, Ryan walks in to the house and sees Heather dying, Ryan begins to cry, he turns on Leatherface and prepares to kill him but Leatherface tells him it was Farnsworth's fault, Farnsworth then grabs the chainsaw and stabs Leatherface in his chest with it, fatally wounding him. Leatherface then begins to die, Officer Hartman and Officer Hooper arrive in the house that was burnt down thirty years ago, they pull out their guns, they tell Farnsworth to put his weapon down or they will open fire, Farnsworth tells them "why should I, why not just kill you for trying to kill my cousin for, huh? Hartman tells him to put the fucking gun down right now, he still doesn't listen, Leatherface still having a little strength throws a meat clever at Farnsworth, Farnsworth then screams and throws the knife at Hooper's chest, killing him. Hartman then opens fire on Farnsworth, shooting him several times, he then shoots him in the head, finally killing him. Leatherface explains to Hartman why he killed people, Hartman then tells Leatherface i thought you would bad, Leatherface tells him i was at once i wish i died those years back, i'm sorry for stabbing Heather. Heather tells him it's okay, you didn't mean it, Leatherface looks at Hartman and says to him "turn on the chainsaw and put me out of my misery, just do it. Heather looks at Leatherface and says to him "I love you cousin. Leatherface tells Ryan "he is really sorry. Heather then turns the chainsaw on and the chainsaw spins around, killing Leatherface. Heather then starts to tear up a little, Ryan, Heather and Hartman all walk out while the police arrive, the place burns down and it is destroyed.

- Marilyn Burns and Gunnar Hansan, who played Sally Hardesty and Leatherface in the original film, make cameo appearances. Marilyn Burns will be playing the grandmother of the Sawyer Family named, Verna Carson and Gunnar Hansan will be seen in archive footage as Leatherface in the ending of the original film.
