Supernatural: All Hell Breaks Loose is a 2021 American dark fantasy horror film. After the car crash, Sam, Dean, Mallory and John end up in the hospital. One of them ends up dead. Sam and Dean kill Azazel, along with old friends and new friends, once and for all.

Plot

The film starts off with the possessed truck driver climbing out of his truck and walk over to the Impala. Sam wakes up inside just as the possessed man pulls the door right off the car. "Get back," says Sam. "Or I'll kill you, I swear to God." "You won't," the demon responds. "You're saving that bullet for someone else." Sam pulls back the hammer. "You wanna bet?" The demon smiles, but a couple of seconds later, it leaves the man's body. Sam tries to talk to John, but he doesn't answer. He then calls Dean and Mallory, but they don't respond either.

Paramedics arrive and take them to the hospital. Dean wakes up, only to see his body lying on the hospital bed still, attached to many tubes and machines. Meanwhile in John's room, Sam and John have an argument. Mallory says "Are you kidding me right now? Do you both really need to argue? Dean is hooked up to a machine and you both are fighting. Grow up." Mallory storms out. Before Sam leaves to get the list of stuff from Bobby, he asks John if he knew what he meant when the demon said he has plans for him and others like him. John says he doesn't. Dean knows John is hiding something.

A woman dies from some kind of supernatural creature. Dean tries to tell Sam there is something in the hospital. Sam is furious at John and tells him if he thought he wouldn't find out. Mallory asks what it was and Sam says that the stuff John wanted isn't for protection but to summon the demon. He tells John he cares more about the demon than Dean. John gets mad as well. Dean tries to stop them but they can't hear them. John tells Sam he's trying to help Dean. Sam asks him how revenge is going to help Dean. They continue to argue, while Mallory tries to stop them, and Dean gets so upset he manages to knock a glass of water off the table. Sam, Mallory and John stare at it in surprise. Suddenly, Dean starts to flicker and he falls to his knees, he's obviously in pain.

In Dean's room, Dean's heart has stopped and they're trying to get him back. Sam and Mallory watch from the doorway with tears in their eyes. Dean walks up behind them. He sees the same white shape he saw in the hallway hovering over his body. He goes forward and grabs the thing, and even though he is thrown away from it, it moves away from his body and his heart starts again.

Dean goes out into the hall and looks for the white thing, but it's gone. He finds a woman named Tessa in the hallway, who is trying to get the attention from people. Dean and Tessa can only see each other. Dean tells her it's an out-of-body experience. He explains how he thinks it's happening to them, and if it is, it means that they're spirits of a body very close to death. Tessa asks if they're going to die and he says no.

Sam and Mallory use a Ouija board to get in contact with Dean, asking him several questions. Sam guesses Dean is talking about a reaper. Sam says if it's there naturally, there is no way to stop it. Panicking, he says there has to be a way, and he goes to find his dad. But John's bed is empty.

John goes downstairs to the boiler room. He starts to draw a symbol on the floor in chalk. Sam goes back to Dean's room telling him he has dad's journal. Dean thanks them for not giving up on them. Sam goes to the page on reapers and the three of them read it. Dean suddenly leaves.

Dean confronts Tessa, figuring out she is a reaper, because reapers can alter human perception. He says they can make themselves appear however they want. She says she was wondering when he'd figure it out. She tells him it's his time to go.

Meanwhile in the hospital's boiler room, John is chanting in Latin. He performs a ritual to summon the yellow-eyed demon. He finishes it and stands up, looking around. The demon comes, possessing a worker. John aims the colt at him. Two other possessed doctors come as well. The demon says he's surprised John summoned him, as he didn't take John as suicidal. He asks John if he really thought he can trap him. John lowers and uncocks the gun and tells him he doesn't want to trap him this time, he wants to make a deal. Tessa is about to take Dean's soul but the yellow-eyed demon takes over Tessa's body and says "Today's your lucky day, kid."

Dean wakes up and doesn't remember anything, except the pit in his stomach. He says something's not right. John says his last goodbye to Dean, Sam and Mallory, without them knowing. He leans down and whispers something in Dean's ear. Then he leaves the room.

John puts the Colt down and says "Okay."

Sam is walking down the hall. Suddenly he sees his father lying down on the floor in his room. He runs in and calls for help. The doctors try to get John back as his sons and Mallory watch, but they can't. One calls, "Time of death, 10:41 am."

After their father's death, Sam and Dean try to deal with it and continue to hunt down the yellow-eyed demon known as Azazel, who caused the fire that led to the deaths of their mother over 20 years ago and later, Sam's girlfriend, Jessica Moore.

While dealing with cases involving normal supernatural creatures, such as monsters and ghosts, the boys have assistance not only from their friend Bobby, who has now become their surrogate father, Rebekah Carson, Ben Mitchell, and Lucy, but also from new allies Ellen Harvelle (who has a history with John), her daughter Jo, Ash, Jenny Wilson and Scott Canning. Part of Azazel's master plan is eventually revealed as he gathers Sam and others like him, leading to Sam's death. Seeing no other choice, Dean makes a deal with a crossroads demon to resurrect Sam in exchange for his soul, which will be collected in one year. In a final confrontation, in which Dean kills Azazel with The Colt using the last of the original bullets, a gate is opened, releasing hundreds of demons from Hell and Dean is facing the ticking clock until his contract is done.

Sam and Mallory try to save Dean from his deal and track down the demons who were released from Hell. Along the way, the brothers and Mallory meet Ruby, a black-eyed demon who also has an interest in Sam and claims to be able to help save Dean. Also, they meet Bela Talbot, an acquirer and seller of occult objects, who is constantly a thorn in their side. The brothers eventually learn what demon holds Dean's contract, a white-eyed demonic being of great power named Lilith, the very first and oldest demon in creation. The brothers and Mallory, along with Ruby, track her down and attempt to kill her. Lilith is unable to stop Sam due to his demonic powers, however. Dean's contract then expires and his soul is taken to Hell.
