A year after the events of the previous film, Perseus and Andromeda are married. They decide to visit the Stygian Witches to find out where Medusa is. Hades plots to release Kronos from Tartarus to get revenge on Perseus and Zeus, but to do that, he needs to drain Zeus' power.

Perseus, Aeson, Andromeda, and Pegasus enter the Underworld to find the Witches, but are attacked by Hades, who nearly kills Perseus. Zeus intervenes to save his son, and is captured by Hades. Andromeda gives Cerberus a steak and he allows them to enter. The Witches make a deal and take Andromeda's tiara before revealing Medusa is on the island of Seriphos. The Witches teleport them out of the Underworld and are killed by Hades, who makes Cerberus one of his warriors.

The group travels to Seriphos and Perseus plans to have Medusa petrify Hades so they can rescue Zeus, whose immortality is slowly being drained into Hades' staff and channeled into Kronos in order to restore the Titan's power. Perseus meets Medusa in Seriphos' forest and discovers she has blinded herself and has tried to commit suicide many times, but can only be wounded by Godly weapons. Perseus tries to convince her to help him, but she firmly believes she will always cause more harm then good. Andromeda has a "girl talk" with Medusa and convinces her to help them.

The group starts returning to the Underworld, but are attacked by a chimera sent by Hades. Medusa and Perseus kill it and begin to bond. On Mount Olympus, Hera discovers what happened to her husband and prepares an assault on the Underworld. Pegasus goes on a scouting mission and discovers the entrance to the Underworld has been sealed off.They decide to seek help from Poseidon, as he can transport them to the Underworld. They sail toward Poseidon's palace, but their boat is destroyed by Harpies. The group is rescued by Poseidon, who gives them respite for the night, but refuses to help as his son, Theseus, was killed by Hades. Medusa repeats what Andromeda said to her to Poseidon, motivating the God to help them.

Hades wins his fight with Poseidon and drains some of the latter's immortality, freeing Kronos. The Titan manifests as a giant made out of lava and ash and creates Harpies to kill the heroes, but Hera leads the Gods into the Underworld and they battle Kronos while Perseus duels Hades. Perseus is fatally wounded by Hades, but manages to decapitate the God. Cerberus saves Andromeda, Aeson, Medusa, and Pegasus from the Harpies and the other Gods destroy Kronos' body by combining their magic and Kronos' remains fall back into Tartarus, trapping him again. Zeus saves his son from death by making him a God, and makes Andromeda and Medusa gods as well. Andromeda tells Perseus she is pregnant, and Medusa marries Poseidon.
