Plot

The movie begins after a mysterious man throws a ORION virus capsule on the ground. The virus liquidates from its crystalline form and turns into a gas spreading throughout the Site. As a quarantine, the facility's artificial intelligence, the Dame (Mackenzie Foy) seals the facility and kills everyone inside. On the outside at the secret entrance to the Site in a mansion in Salem on the outskirts of New York City.

Sarah Winter (Brie Larson), the head of security at the Site and the secret entrance, fell unconscious from the nerve gas and suffered from memory loss including all of her memories of GenCorp. Shortly after, an environmentalist, Michael Ayo (Chiwetel Ejiofor), gets into the mansion about the same time GenCorp's biohazard team led by Commander Bishen Akbar (Irrfan Khan) breaks in. They quickly apprehend the two and move to the underground train leading to the Site. After restoring the train's power and moving toward the Site, Lance Corporal Fen Qiang (Ken Leung) opens the door to the next car where a man named James Marcus (Tom Hiddleston) collapses and is introduced. He suffered from the same nerve gas Sarah was introduced to.

The group quickly makes it to an experimental storage room where Sarah, Michael, Marcus, Corporal Chen Wu (Reggie Lee), Gunnery Sergeant Ana Marques (Alice Braga) and Lieutenant Mauro Silva (Diego Luna) stay. The rest of the group advances to the Dame's chamber. Silva shuts down the security systems so they can proceed.

Akbar, Lance Corporal Martha Simmons (Rosalind Halstead) and Private First Class Eren Björk (Arben Bajraktaraj) proceed through the hallway, where they are sealed by the Dame's security system. Akbar was the first to see the laser, but as it came it ended Martha's life as well, by decapitation. As the Laser passed by a second time, Eren tried to dodge it promptly by jumping, the laser promptly killed him as well by slicing him through the torso. Finally as the final laser came through, Akbar died as the laser turned into an impossible to dodge grid, cutting him into dozens of pieces. Silva attempts to deactivate it, but does not succeed until after everyone is dead. Shortly thereafter Silva and Sarah proceed to shut down the Dame. Because of the shut down, all electronic units, including the doors and seals trapping the infected people inside are opened.

As this happens, Ana and Chen come across a infected. Not knowing it was actually a mutated human, it bites Ana's hand, infecting her. Chen and Ana are quickly rattled by its persistence to move even after several rounds of bullets were fired at it. Sarah, Michael, Silva, and Marcus return to Ana and Chen afterwards. During this time they are surrounded by countless of infected and the group uses most of its ammunition. Chen is separated from Ana and is devoured by a horde of the infected workers.

Ana, Silva and Marcus return and barricade themselves in the Dame's chamber. Michael enters the offices searching for his sister Zoe only to find that she has been infected. Sarah saves Michael and kills Zoe, who was about to devour her brother. It is then that Michael explains why he's there. His sister, Zoe, had infiltrated the Site and had a contact there to steal the ORION virus and expose GenCorp to the world. Zoe was supposed to use this contact to get the virus and exit the Site and give the virus to Michael so he could expose GenCorp.

Sarah and Michael return to the Dame's chamber. After being boxed in with no escape, Sarah reactivates the Dame and keeps her on remote activation for insurance. In doing so, they were able to blackmail the Dame for info on the virus and a way to escape.

They exit through storage tunnels where they are attacked again. Ana is then infected for the third and fourth time since the first undead encounter. Silva is separated from the group, as he destroys a passage in order to allow the rest of the group to live. Sarah, Michael, Marcus and Ana reach the surface afterward. Ana by this time has been infected on a massive level and all of her senses are impaired including lack of strength to walk. About that time, Sarah recalls about the Anti-Virus, the cure to the ORION virus. They enter the experimental room to recover it only to find it missing. Marcus then regains his memories and it is revealed that he was the one who let the virus go free. He would then use the remaining capsules on the black market and make himself a rich man. When Sarah refused to go with him he leaves the room holding everyone at gunpoint and seals the exit.

As he returns to the train to surface, he is killed by one of the Site's early experiments with the ORION virus called the Walker. The Dame didn't tell them of its escape because she hadn't expected anyone to survive that long without becoming infected. It was her back-up plan. The Walker then finds them and tries to break in while the Dame agrees to give them access to leave if they kill Ana. Even with the Anti-Virus on the train, the ORION virus had infected her on a massive level and there was no guarantee that the Anti-Virus would work. Silva then returns and shuts down the Dame, opening the door so that they can escape as the Walker enters from the opposite entrance. They make it to the train and travel to the surface. Ana is given the Anti-Virus, and Silva is as well, but the Walker returns and kills Silva, as well as infecting Michael. The Walker turn and targets Sarah, but she manages to secure its tongue into the ground with a pipe while Michael goes to open the storage hatch it's standing on. Ana, who has been turned into a infected, attacks Michael, but he is able to kill her swiftly as she falls on the hatch mechanism and the Walker is caught on the tracks, being ignited and left behind to burn.

At the mansion, Michael's wound begins mutating. Before Sarah can give him the Anti-Virus, the mansion doors burst open and a group of GenCorp scientists, led by Doctor Alan Whistler (Gary Oldman) seizes them. They subdue Sarah and take Michael away, revealing he is to be put into the Underworld Program.

One week later, Sarah awakens at Brooklyn's Metro-General Hospital, strapped to an examination table, with no memory of what happened since her capture. After escaping, she goes outside to find New York City abandoned and ruined. Sarah arms herself with a shotgun from an abandoned police car as the camera pans out.
