In 1964 disturbed child Malachai Boardman, led by friend Isaac Chroner, murders his mother, abusive older brother and neighbours, before ending the boys' small rural town massacre with Isaac's bound parents, as various other children gather around.

12 years later couple on the verge of divorce: Vicky and Burt are travelling to Grand Island, Nebraska when taking a shortcut through a seemingly abandoned small town highway near a cornfield they collide with an already injured young boy. The boy has a visible slit throat that is bleeding out and the couple bring the boy into the car, planning to drive him to the closest hospital.

The couple arrive in the close ghost town of Gatlin, Nebraska which is void of any people, until they hear chanting from the church, where a cult of children, led by Isaac are about to drain the blood of two young children Job and Sarah. Burt and Vicky intervene, saving the two children and are having their car damaged are forced to hide within the hidden basement of Job and Sarah's family home. It is revealed the dead boy in their car is named Joseph, and like them was the a defector of the cult, who had attempted to flee.

A demon eventually appears in the underground tunnels of the basement and drags away Burt as Vicky attempts to flee with the children, in what appears to be her last moments with the two children upstairs, she uses the home phone to call the police.

Responding Sheriff Skaggs aligns with Vicky and Burt's happily married couple of friends Sarah and Mitch York who following their friends' failure to arrive at theirs in Grand Island have hired a local guide Frank to help in finding them. They come to Gatlin and spend the day going across the township, uncovering the many undiscovered corpses of all it's adult denizens, and many of it's child inhabitants, as well as the re-animating corpse of Joseph, which climbing from the wreckage of Vicky and Burt's car they find in the cornfield attacks and fatally wounds Skaggs.

At nightfall as the group are unable to leave with their respective vehicles damaged, the cult descends on them, Mitch and Skaggs both murdered ritualistically: as Malachai drowns Skaggs in his own blood and Mitch is immolated in the cornfield as a devastated Sarah rushes to him. Frank wards off the attacking children before Malachai despite Isaac's protests slashes him to death with a sickle. Little Job and Sarah still alive, come upon Sarah at the site of her husband's burning body and escort her to safety, along the way passing the ritualistically displayed, and mutilated corpses of Burt and Vicky.

Malachai turns on Isaac who confronts him before the others and is challenged with being incompetent in his leadership, as Malachai protests his service in the means of killing since the beginning, with each member having to murder their own parents, that along with the siblings Job & Sarah and defector Joseph, he himself unlike the others did not serve their god by offering up their own blood.

With this Malachai slashes into Isaac's skull with his sickle, bringing forth the roar of their god, which he refers to as 'He who walks behind the rows' and he seems to in a fashion similar to Burt Stanton be consumed by the entity.

Coming to the cornfield barn of Isaac's parents the child siblings hide the adult Sarah, until they are forced out by the looming force of the entity. At a fiery altar awaiting outside, a hobbled Malachai is overseen by a re-animated Isaac, possessed by the god which in a booming voice confronts him and disintegrates him, absorbing his flesh. The god Isaac then welcomes Sarah, who will be the final and most important sacrifice, the one that will bear life (as unbeknown to her) she is pregnant. With the siblings help Sarah is able to fend off the god, destroying the vessel of Isaac with Malachai's sickle and her escaping onto the road, where she like Joseph is hit by an oncoming vehicle.

The driver a mother and her two kids escort her to safety.
